Does Insurance Cover Aquablation?What Patients Need to Know

For many men, the moment they hear about Aquablation — a robotic, heat-free, minimally invasive treatment for BPH — the very next question is: Will my insurance cover it? It's a fair question, and an important one. The short answer is: yes, in most cases. But as with anything involving insurance, the details matter.

Is Aquablation FDA-Cleared?

Yes. Aquablation therapy received FDA clearance in 2017 for the treatment of symptomatic BPH in men with prostates ranging from 30 to 80 cc — and that clearance has since been expanded to include larger prostates. FDA clearance is the baseline that insurance companies look for when evaluating whether a procedure is eligible for coverage.

Does Medicare Cover Aquablation?

Medicare — which covers millions of American men over 65, the age group most commonly affected by BPH — does cover Aquablation therapy. The procedure has established CPT (Current Procedural Terminology) billing codes, which means it can be billed and reimbursed through Medicare like other standard urological procedures.

If you are on Medicare, coverage typically includes the facility fee, the surgeon's fee, and associated anesthesia costs, subject to your plan's standard deductibles and cost-sharing provisions.

What About Private Insurance?

Coverage under private insurance varies by plan, but Aquablation is increasingly recognized by major commercial insurers. Because it is FDA-cleared and supported by substantial clinical evidence — including long-term data from the WATER and WATER II trials — many major insurance providers have added it to their covered procedure lists.

That said, "covered" does not always mean "covered in full." Your out-of-pocket costs will depend on:

What Is Prior Authorization and Do I Need It?

Prior authorization (PA) is a requirement by some insurance plans that your physician obtain approval before performing a procedure. It's essentially the insurer confirming that the treatment is medically necessary for your situation before agreeing to pay for it.

Many BPH procedures, including Aquablation, require prior authorization. This is not a reason to worry — it's a standard step our office handles on your behalf.
