"Success rate" means different things to different people. For BPH, it comes down to four questions: Do symptoms improve? Does flow improve? Does it last? And what did you give up to get it?

Symptom Score Improvement

The IPSS score runs 0–35. Men entering Aquablation trials typically scored in the low-to-mid 20s — severe territory. Post-procedure scores dropped into the mild range and stayed there through long-term follow-up. That's the difference between organizing your life around bathrooms and not thinking about it.

Urinary Flow

Peak flow rate roughly doubled in study populations, with corresponding drops in post-void residual volume — meaning the bladder empties more completely, which is what drives improvement in nighttime frequency and urgency.

Durability

Five-year data show the improvements hold. Surgical retreatment rates remained low and compared favorably with TURP, which has long been the durability benchmark.

What You Don't Give Up

This is the differentiator. Resective procedures deliver strong flow results, but TURP carries retrograde ejaculation rates commonly cited at 60–90%. Aquablation's rates are dramatically lower, and erectile function is generally preserved.

Who Does Best

• Men treated before the bladder has been damaged by years of obstruction
• Men whose surgeon has high case volume
• Men who follow post-op instructions in the first two weeks
